EARLY IDENTIFICATION

& INTERVENTION SERVICES

Early Intervention builds on the natural learning occurring during the first few years of life. The support and services provided through early intervention can help you to help your child make all the powerful connections to improve his or her ability to learn and play. Hearts & Lives, in partnership with the County of San Bernardino Department of Behavioral Health and First 5 San Bernardino, will provide activities targeted to improve social, developmental, cognitive, emotional and behavioral function through services as they relate to Early and Periodic Screening.

Necessary interventions will be put in place in our program to help facilitate optimal development. Parents and caregivers will be given the support and guidance that they need to promote continuum of care at home and in school.

 

Presenting problems may include:

  • Aggressive Behavior

  • Speech Problems

  • Short Attention Span

  • Prenatal Exposure to Drugs and/or Alcohol

  • Infants that are difficult to comfort

  • Sleep difficulties

  • Excessive Tantrums

Why is EIIS important

These early years are the prime time for your child’s vision and hearing, speech and language, physical and motor, and social-emotional developments.

Who qualifies for the services and Discharge Criteria:

All children from birth to their 9th birthday (with services initiated by their 8th birthday) are eligible for services. A client is discharged after successful program completion, or when he/she reaches the age of 9 years old.

Evaluation and Assessment

Parents/Caregivers will always be a part of the evaluation process, which is critical in determining your child’s strengths and needs, and in planning the best course of action.

Hearts & Lives will help you develop a plan suited to each individual child.

Screening

The Ages and Stages Questionnaire 3 (ASQ-3) and the Ages and Stages Social Emotional (ASQ-SE) are used as an initial screening tool to evaluate the child’s current level of functioning.

ASQ 3 screening identifies 5 domains:

  • Communication

  • Gross Motor

  • Fine Motor

  • Problem Solving

  • Personal-Social

Assessment

Based on the results of the screening, a partial or full comprehensive assessment may be recommended.

Referral

Based on the assessments, recommendations and referrals may be made to the parent/guardian of the child.
