CASE MANAGEMENT

Hearts & Lives Case Management Program links community members of all ages with services they require with the ultimate goal of keeping a healthy and safe community.

Case Management focuses on assisting community participants to set and reach achievable life skills goals, as well as provide information and linkage to available services.

It begins when a person or family is identified as having a vulnerability or is in a difficult situation requiring support or assistance.

Hearts & Lives case worker collaboratively assesses the needs of a participant or a family and arranges, coordinates, monitors, evaluates and advocates for a package of services to meet their specific needs. This may require the case worker to liaise with and involve community members and leaders, non-governmental and faith-based organizations, as well as representatives of education, and health professionals. This collaboration and coordination are very important to ensure that a range of services, as identified in the case plan, are delivered without interruptions and that needs are met
